Poultry Chicks Mortality

Can someone explain to me like a nursery school kid on how to avoid chicks mortality?It seems the hardest part of poultry keeping is ensuring chicks reach the 3 months old mark.
I've been doing vaccinations but they still die,very frustrating

Hygiene first priority. Don't mix flock of different ages.vaccinate in good time

What are the right temperatures?
Also,can age difference of about 3 weeks affect chicks when mixed?
